DataLink brings optimized interoperability to Texas Independence Health Plan with EvokeConnect

Tampa, Fla. (Aug. 20, 2024) — DataLink, a leading provider of data-driven solutions for high-quality, value-based care, today announced that they’ve partnered with Texas Independence Health Plan, Inc., a Medicare Advantage HMO Institutional Special Needs Plan (HMO I-SNP) for individuals enrolled in Medicare who reside in long-term care facilities.

TIHP will be using DataLink’s EvokeConnect solution to efficiently perform chart chases as part of its risk adjustment and quality program.

“DataLink is excited to be working with TIHP, an organization known for offering top-notch care for residents in nursing homes, skilled nursing facilities, and other long-term care facilities,” said Ashish Kachru, CEO, DataLink. “We know that our EvokeConnect solution is just what they need to optimize interoperability, further improving the care for their members.”

EvokeConnect streamlines clinical data collection and management, allowing clients to access richer, more accurate data for true data interoperability. It offers one chart, one charge per year, reduced administrative burden, prospective gap closure, and enhanced safety and security with our HITRUST certification and SOC2 Type 2 attestation.

“We’re looking forward to collaborating with DataLink, and we’re confident that it will yield incredible results for our risk adjustment and quality initiatives,” said Devan Dekowski, CEO, TIHP.

TIHP launched the I-SNP in 2020 to provide the best possible care for long-term care residents. The leadership team has decades of experience delivering the highest quality long-term care as well as a history of developing innovative programs that improve care and keep patients healthier.

DataLink is connected to over 95% of EHR platforms, enabling the rapid retrieval of clinical data required for gap closure and improving patient health. The DAV-certified solution streamlines the auditing process and reduces costs by reducing the necessity for Primary Source Verification.
